This is a sample letter to the Head of a State Archive in Poland. The contents are translated using simple text with additional comments shown in italics.

Date of marriage: Jun. 19th, 1845
Locality: Trzcinica, district of Konin.

Words to be substituted for "marriage" in the letter (death, etc,) are provided.
Defining your relationship to the person whose certificate you are applying for is required. Additional words of kinship in Polish are: ojciec (father), matka (mother), babka (grandmother), prababka (greatgrandmother), dziadek (grandfather), pradziadek (greatgrandfather).
Please pay strict attention to the Polish diacritical marks!
